Join us for the AFFIRMO Final Conference, where leading experts will discuss how integrated, evidence-based and patient-centred care pathways can improve the management of atrial fibrillation in older adults living with multimorbidity, frailty and polypharmacy.

The conference will present the main findings of the AFFIRMO project and explore their implications for clinical practice, healthcare policy and the future of integrated care. Key topics include multimorbidity, clinical complexity, polypharmacy, health system sustainability and patient empowerment.

The programme will also feature a multidisciplinary panel discussion on future guidelines and care pathways, bringing together representatives from AFFIRMO and other European initiatives working in atrial fibrillation, digital health, personalised care and AI-supported decision-making.
